New Patient Registration

Before completing a registration form, please check you live within our boundary by checking on this map - practice area. We unfortunately cannot accept registrations outside of this area.

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You can collect the forms at either surgery or register online using the following link.

Please note that while there are separate sites, you will be registered with North Preston Medical Practice and appointments will be offered at both sites when you contact the practice.

The Practice recommends that all patients should attend for a New Patient Medical. If you are not on any medication and do not wish to have a New Patient Medical you are given the option to opt out. Please bring a list of your medication and a urine sample to your appointment should you wish to attend. Those patients on medication are advised to attend for a New Patient Medical. 

Please note: if you are completely housebound and unable to attend the surgery, please inform the receptionist, who will make alternative arrangements for you. 

We take your care very seriously, so failure to book or attend the New Patient Healthcheck will mean we will not be able to continue with your application to join the practice.

Medical treatment is available from the date of registration. Please contact reception for further information.

New patients please note that some of our services such as Family Planning and Minor Surgery are provided from our Ingol Health Centre site.

Unfortunately, we are unable to accept applications for patients residing outside of our practice boundary. Existing patients moving outside of our practice area will be required to re-register with another practice that covers their new address.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
